Release of Liability, Assumption of Risk, and Indemnity Agreement Please read carefully. This Release of Liability, Assumption of Risk, and Indemnity Agreement ("Agreement") contains a waiver and release of certain legal rights. I acknowledge that I am the legal owner or authorized agent of the dog(s) receiving services from Mile High K9 Massage & Splash ("Company"). I voluntarily choose to allow my dog(s) to participate in canine massage, swimming, fitness activities, sensory enrichment, and/or use of the Company's equipment and facilities (collectively, "Services"). Assumption of Risk I understand that participation in the Services involves inherent risks including, but not limited to slipping, falling, fatigue, muscle soreness, panic responses, water exposure, equipment use, and unpredictable animal behavior such as scratching or biting. I knowingly and voluntarily assume all risks, both known and unknown. Dog Health & Behavior Acknowledgment I affirm that my dog(s) are physically able to participate in the Services and do not have uncontrolled seizures, contagious conditions, or cardiovascular limitations that would make participation unsafe, unless such conditions have been fully disclosed to and expressly approved by the Company in writing. I understand it is my responsibility to consult a veterinarian if I have any concerns regarding my dog's participation. I accept full responsibility for my dog's behavior and actions at all times. Release of Liability To the fullest extent permitted by Colorado law, I hereby release, waive, and discharge Mile High K9 Massage & Splash, including its owners, officers, employees, contractors, and agents, from any and all claims, demands, causes of action, or damages arising out of or related to participation in the Services, including claims arising from the ordinary negligence of the Company. Indemnification & Hold Harmless I agree to indemnify, defend, and hold harmless Mile High K9 Massage & Splash from any claims, liabilities, damages, costs, or expenses (including reasonable attorney's fees) arising from my dog's participation in the Services, my dog's behavior, or any breach of this Agreement. No Guarantee of Results I understand that no guarantees are made regarding results, outcomes, or improvements from massage, swim, or fitness services. Owner Acknowledgment I have read, understood, and agree to the terms above. I am at least 18 years of age and I am the owner or authorized agent of the dog(s) receiving services.

Pool Policies • Do not feed your dog within at least 3 hours of a swim session. • Please ensure your dog has gone potty prior to any swim session. • Please brush your dog before arriving to help protect pool filters and water quality. • No individuals under 18 years of age are permitted on the ramp, deck, or in the pool area. • Owners are not permitted in the pool unless prior permission is granted and a waiver has been signed. • If your dog is sick, has diarrhea, loose stool, or shows signs of illness, please do not attend swim sessions. • Puppies are permitted in the pool after 12 weeks of age and once all age-appropriate vaccinations have been completed. • In the event your dog has a bowel movement in the pool, a $200 sanitation fee may be charged, as the remainder of the day's schedule may need to be canceled for cleaning and safety purposes.
